Centralized Admissions Coordinator

Are you proficient in nursing home admissions, but want a break from the healthcare setting? Foundations Health Solutions, Ohio’s largest skilled nursing and rehabilitation provider, is hiring a full-time Centralized Admissions Coordinator to our Centralized Admissions Department.

The person in this position will work from our Mentor, Ohio, location during regular business hours (8:30 a.m. to 5:30 p.m.) for part of the week. The other part of the week can be worked from home, if the hired candidate wishes. Occasionally, this position will require you to take call on evenings and weekends.

Your basic math skills and ability to identify healthcare costs would be used to review referral information from our hospitals and community referral sources and successfully prepare our facility-based admissions directors to admit and care for the patient. The clerical aspects of the position require you to be familiar with clinical records, pay close attention to detail, and be self-driven to execute processes that move the referral through the admission process.

The ideal candidate will have:

• Excellent written and spoken communication skills
• The energy needed to continuously process referrals at a fast pace
• Experience working within a skilled nursing facility
• At least one year of experience in the nursing home admissions process or be familiar with Medicare, Medicaid, and other types of insurances to work through the admissions process
• A positive attitude and approach toward the job, colleagues, and customers
